Use the huge waterwheel to go down here (about halfway to the bottom).

To get to the room where the chest is, you have to climb the crystals in the room with lots of them to reach the secret passage and then jump down.

At the bottom of the Giants' Mountaintop Catacombs. Hint: The elevator is "bi-level". When you go down near the door to the boss room then send the elevator up.

In Auriza Hero's Grave. To get here you will have to jump down onto the narrow beams at some point.

It will be left by the spirit you find here as long as you have looked at the "Prophecy" Painting beforehand.
